Indiana law requires the following immunizations for school attendance:
Kindergarten and Grade 1:
5 DPT, 4 Polio, 2 MMR, 3 Hepatitis B, 2 Varicella
Grades 2- 5:
5 DPT, 4 Polio, 2 MMR, 3 Hepatitis B, 1 Varicella
Grades 6-12:
5 DPT, 4 Polio, 2 MMR, 3 Hepatitis B, 2 Varicella, 1 Tdap, 1 MCV4
To legally attend school, please see that your student receives these shots from their physician or the health department and bring the shot record to the school nurse or guidance department BEFORE your student attends the first day of school.
Free shots from the Tippecanoe County Health Department (TCHD) can be obtained by:
1) Calling the TCHD at 423-9222 to make an appointment to have shots administered any Tuesday or Thursday, or
2) Attending a walk-in clinic the 1st or 3rd Wednesday of each month
There will be NO school-based shot clinics this year.
